Certificate of Attendance Orders

CASIE provides one complimentary Certificate of Attendance at the close of each workshop. Additional copies are available for $15 each. To order your copy, please follow the instructions below:

• Download the order form by clicking here.
• Complete the order form
• Enclose your check or money order made out to CASIE for $15
• Mail to CASIE, Attn: Certificate Copies, 2890 North Fulton Drive, Atlanta, GA 30305 USA

Please allow 6-8 weeks for processing. Orders must include payment in order to be processed. Any orders received without payment will not be processed.
